Action required

Verify your spouse's eligibility.

Only spouses without access to other coverage — an employer plan, retiree coverage, or Medicare/Medicaid — are eligible for the Lakeland medical plan. If you cover a spouse, you must complete verification within 30 days of your hire date, or their coverage will end. Lakeland's advisor, Paragon, administers the process securely.

Questions People Ask Most

When can I enroll or make changes?

During annual Open Enrollment, within your new-hire window, or within 30 days of a qualifying life event — marriage, birth or adoption, or gaining/losing other coverage. Submit changes and documentation in Benefitsolver within 30 days, or you'll wait until the next Open Enrollment.

How do I add a dependent?

Log in to benefitsolver.com (company key lakecounty), choose Life Events > Update, and upload supporting documentation within 30 days of the event. Have dependents' dates of birth and SSNs handy.

Where is my ID card?

Digital cards live in the MedMutual app for medical/dental and the TrueRx+ app for pharmacy. Download either from the App Store or Google Play and log in or create an account.

How much does Teladoc cost?

Teladoc visits are subject to your medical plan's deductible and coinsurance. Register at TeladocHealth.com or call 1-800-835-2362, then request a visit by phone or video. Review your plan details for the cost that applies to you.

Is the EAP confidential?

100% confidential. AllOne Health is an independent partner — your district never sees who calls or why. You get up to 6 free sessions per issue for your whole household. Call 800-451-1834 (company code LCSCeap).

What is the opt-out incentive?

If you have qualifying medical coverage elsewhere, you may be eligible for an annual opt-out payment of $5,000 for single coverage or $10,000 for employee/family coverage. Contact HR at (440) 525-7045 for eligibility requirements and enrollment details.
